Job Summary:
The Housing Case Manager is responsible for providing case management services to individuals who are experiencing homelessness. The Housing Case Manager will work with clients to identify their housing needs, connect them with resources, and develop individualized plans to secure and maintain permanent housing. This position requires a high level of empathy, problem-solving, and understanding of housing-related issues.
Job Hours/Work Schedule: The position requires availability for evening appointments two nights a week, typically until 8:00 PM. In addition, the company is open to accommodating a four-day workweek (instead of the standard five-day schedule).
Key Responsibilities:
Assess client needs and eligibility for housing assistance programs.
Develop and implement individualized housing plans with clients.
Assist clients in finding and securing permanent housing options.
Maintain accurate and up-to-date client records, case notes, and documentation in accordance with program requirements.
Advocate on behalf of clients to secure housing and resolve barriers to housing stability.
Monitor clients' progress and make necessary adjustments to housing plans.
Conduct outreach as necessary.
Prepare and submit reports on client progress, services provided, and outcomes.
Ensure compliance with applicable policies, regulations, and ethical standards.
