Essential Job Functions/Responsibilities

  • Receives and appropriately enters/documents new referrals in a timely manner.
  • Completes workflow tasks and other assignments to the IC Responsible Position in the Agencys electronic medical record.
  • Participates in coordinating care with nurses, patients, and the interdisciplinary team.
  • Provides effective communication to patients, staff members, other healthcare professionals and referral sources.
  • Responds to Agency and patient needs in a professional and creative manner.
  • Demonstrates commitment, professional growth, and competency.
  • Participates in Agency-sponsored in-service training.
  • Participates in the Agencys QAPI program.
  • Promotes the Agency philosophy and administrative policies to ensure quality of care.

Position Qualifications

  • One year experience in hospice or healthcare is preferred.
  • If clinical, current state license as a licensed professional or vocational nurse.
  • If clinical, graduate of an accredited school of vocational/practical nursing. If non-clinical, must have a high school diploma or equivalent.

Experience to Deliver on our Mission:

  • Current RN License, valid state driver's license, and reliable transportation
  • One year of experience as a Registered Nurse in an acute or clinical care setting preferred. Home health experience preferred
