Job Objective:

Under the direction of the Director of Case Management, or designee (Manager), the LCSW Social Work Case Manager is responsible for evaluating, assisting and developing a discharge plan for patients and their families who have complex bio-psycho-social needs. The LCSW Social Work Case Manager is expected to function well as a member of the interdisciplinary team and participate in the referral development process.

Duties and Responsibilities:

  1. LCSW Social Work Case Management
  2. Completes bio-psycho-social assessments, develops a treatment plan for referred patients and documents in the hospital medical record according to department policy.
  3. Assesses for Home Health and DME needs, makes referrals to appropriate agencies and documents in the medical record to facilitate the completion of arrangements.
  4. Communicates the treatment and discharge plan to the healthcare team.
  5. Participates in multidisciplinary rounds and trauma rounds.
  6. Initiates culturally sensitive clinical interventions and short term counseling with input from the interdisciplinary team.
  7. Provides culturally sensitive education to enhance the patient and family's understanding of diagnosis, care plan, ability to cope with illness or injury and the continuum of care.
  8. Participates with patients, their family, the Nurse Case Manager and/or Case

Management Assistant in Post-Acute Care Planning involving complex bio-psycho-social needs.

  1. Facilitates transitions to alternate levels of care.
  2. Researches, communicates and recommends alternative cost effective health care services to the health care team.
  3. Provides Crisis Intervention.
  4. Educates patients and their family about resource options and how to access community resources.
  5. Serves as a patient and hospital advocate regarding all clinical, social, financial and ethical healthcare matters.
  6. Evaluates cases of suspected child, domestic, adult, and elder abuse or neglect.
  7. Provides consultation to hospital staff and clinical oversight to ensure a safe discharge is arranged.
  8. Identifies and reports abuse of children and adults as mandated by state law.
  9. Coordinates and facilitates adoptions.
  10. Coordinates family conferences.
  11. Provides in-service training to hospital staff, as requested.
  1. LCSW NICU Social Work, As Assigned
  2. Participates in and attends NICU/discharge planning rounds with the Interdisciplinary Team.
  3. Attends pen-natal rounds/case finding.
  4. Completes a comprehensive psychosocial evaluation within 48 hours of admission.
  5. Maintains weekly summaries and completes discharge note as appropriate.
  6. Has working knowledge of CCS regulations.
  7. Maintains ongoing contact with the baby's family during NICU stay.
  8. Coordinates and attends physician conferences with families.
  9. Contributes to utilization management and the CCS referral process.
  10. Must be CCS paneled within 6 months of hire.
  11. LCSW Pediatric Social Work, As Assigned
  12. Assess CCS pediatric patients with LOS of 3 or more days.
  13. Maintain weekly summaries for CCS pediatric patients with LOS of 3 or more days and completes discharge note as appropriate.
  14. Must be CCS paneled within 6 months of hire.
  15. LCSW Clinical Supervision Responsibilities
  16. Provides clinical supervision to Associate Clinical Social Workers registered with the California Board of Behavioral Sciences in an individual or group setting

  • Working Knowledge of InterQual Guidelines
  • Working knowledge of Government, State, HMO, PPO , Commercial, and Workers Compensation utilization, authorization and billing guidelines
  • Working knowledge of Microsoft Office applications
  • Working knowledge of medical terminology

  • Proficient in the operation of scanners, copiers, and fax machines

Education and Experience:

Education

  • Master in Social Work (MSW) from an accredited program by Council of Social Work Education

Experience

  • Minimum of two years experience in Social Work
  • Previous Social Work experience in an acute care setting preferred

Required Licensure and/or Certifications:

  • Active Licensed Clinical Social Worker from the California Board of Behavioral Sciences
  • Completion of California Board of Behavioral Sciences required 15-hour clinical supervisor training
